changing text to hidden on join.html

I have searched very hard and found simple answers for my questions but some answers dont explain very well. I am trying to simply hide fields on the join.html i change the input type to hidden and thats all but when i upload the file and enter info into the fields and click join it does not do anything.

Also i would like to make the field company required but looking through forum does not give very good explanation and all that needs to be changed.

last, I would like to change the country to State and have drop down with all us states instead of countries but any kind of change on join.html clicking join does nothing. any help on these issues would be great! Thank You
